健康診断証明書は証明書自動発行機と保健管理センター（川内北キャンパス）で発行

Health checkup certificates are Issued by an automatic certificate issuing machine　and at Health Service Center (Kawauchi Campus).

　※各キャンパス全７か所に設置(設置場所は東北大学学生生活案内 P14参照)。
　※指定様式がある場合，保健管理センターにお問い合わせください。

　※These are available at all seven locations on each campus (see p. 14 of the Guide to　Student Life at Tohoku University 2022 for locations).
　※If there is a designated form, please contact the Student Health Service Center.
