AY 2024 Regular Medical Checkup
※2024年度の定期健康診断は NET予約が必要となります。
A net reservationr is required the regular medical checkup in 2024.
2024年4月12日から予約開始です。5月7日までにNET予約を完了させてください。
Reservations will start from April 12, 2024. Please complete net reservation by May 7th.
〇 定期健康診断期間以外には健康診断を受けることはできませんので、指定された日に都合の悪い方は、期間中のほかの日時にNET予約の上必ず受けるようにして下さい。
〇 You can only take a health checkup during the regular medical checkup period. If you are inconvenient on the specified date, please be sure to make a net reservation at another date and time during the period.

Important notice !! Instructions for undergoing chest x-rays
〇社会人学生の方へ
For adult students
職場或いは学外で別途健康診断を受診した場合には、健診結果（胸部X線撮影結果必須）の複写を保健管理センターに直接ご提出いただければ、本学の定期健康診断の受診は不要となります。（複写の提出は郵送でも可）
但し、センターから健康診断証明書の発行はできません。また、健診結果の原本を提出された場合は返却できませんので予めご了承ください。
Adult students who underwent a medical exam at their workplace or another off-campus facility may submit a copy of their results (must include chest X-ray) to the Student Health Care Center instead of undergoing the university’s examination. However, in this case the center will be able to issue a Medical Examination Certificate. Be aware that if the original copy of the medical examination results is submitted, it will not be returned.
